Outpatient Psych Svc Admissions Coord, Full Time, Benefited, Days, 8hours, 1.0fte + Oakland, CA + Highland General Hospital + HGH Outpatient Psychiatric Srvcs + Full Time - Day + Business Professional & IT + Req #:41057-30343 + FTE:1 + Posted:December 19, 2024 **Summary** **SUMMARY:** Responsible for acting as a liaison between community mental health agencies and the Alameda County Medical Center outpatient psychiatric services program. Responsible for conducting initial screening of individuals for clinical appropriateness for treatment. Develops program relationships with community providers to enhances community education, integrate the program with the local mental health constituencies and improve the continuum of client care. **DUTIES & ESSENTIAL JOB FUNCTIONS** :NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification. 1. Community Liaison: 1. Develops relationships with various community resources to inform them of the availability of program services. 2. Develops and follows a regular call cycle; Provides coordination of, training regarding and implementation of contact plan. Tracks team outreach and community education/communication activities and productivity. Coordinates or provides contacts with all existing or prospective referral sources. Coordinates appropriate and productive training’s, seminars, site visits etc. 3. Attends monthly AMI/support group meetings; 4. Attends public mental health system planning meetings and other similar events and represents program and client interests; 5. Conducts clinical screenings and follows through with the admission process; 6. Sets up and conducts tours for community resources; 7. Serves as a community resource for educational and community resource information; 8. Attends inpatient treatment program tracks to the community. 9. Introduces new treatment program tracts to the community. 10. Assists Program Director and Medical Director to identify medical staff and community psychiatrist participation. 2. Community Analyst: 1. Develops a quarterly call cycle plan; 2. Compiles and analyzes monthly Program Education Monitors; 3. Develops an annual community needs analysis; and 4. Communicates community resource needs and requests to the Program Director. 5. Assumes responsibility for strategic plans and utilization reports to include trending, utilization tracking, analysis of the community and contact productivity and designing new product lines and development plans 6. Participates in mental health forums to maintain current knowledge of changes within the local mental health system and to identify program opportunities for expansion. 3. Treatment Team Member: 1. Attends staff and treatment team meetings and contributes pertinent information; 2. Participates in the new client orientation process; 3. Follow up on admitted clients to insure engagement with the program; 4. Keeps program personnel informed of community trends, issues, concerns and request; and 5. Shares ideas, successes and challenges with other CLD staff.Performs other duties as assigned by the program administrator **MINIMUM QUALIFICATIONS:** 1.Education: Bachelor's degree or RN License with a minimum of 4 years experience; 2.Experience: 2 years working with seriously ill adults and 2 years working in healthcare related community education or outreach capacity; or a Masters degree in mental health, social work or human services field. **PREFERRED:** Masters degree in Social Work, Counseling, Psychology or related field. **Additional Information** **MINIMUM QUALIFICATIONS:** 1.Education: Bachelor's degree or RN License with a minimum of 4 years experience; 2.Experience: 2 years working with seriously ill adults and 2 years working in healthcare related community education or outreach capacity; or a Masters degree in mental health, social work or human services field. **PREFERRED:** Masters degree in Social Work, Counseling, Psychology or related field.
