Conducts initial and follow up assessments as determined by individualized treatment needs and monitors progress towards treatment goals. Develops, implements and supports a treatment milieu designed to support the specialized treatment needs of program participates in collaboration with other treatment team members. Provides individual, group and family therapy treatment services. Conducts initial and follow up assessments using reliable and valid assessment tools to address the needs of the individual’s service in the program. Participate in the development of creating wellness and resiliency plan that identifies social, emotional and behavioral goals for treatment in collaboration with the program participant. Monitors the progress of each participant toward the achievement of individualized treatment goals. Collect and analyze data to monitor the efficacy of behavior change strategies. Participate in bi-weekly medication review meetings with the treatment team. Provide crisis management services which may include the facilitation of emergency psychiatric hospitalization services, when necessary, under the supervision of the Outpatient Program Director. Complete all required documentation, including, but not limited to progress notes, wellness/resiliency plan and discharge summaries. Participate in discharge planning meetings, support the transition process and provide any necessary documentation to facilitate further communication reintegration. Maintain credentials and continuing education requirements. Perform other duties as assigned. Must have own reliable transportation and a valid driver’s license. Must be able to lift a minimum of 50 pounds. Education Graduate degree in psychology or related discipline. Licensed mental health professional required. Familiarity with evidence-based practices. Certification in select evidence-based practice is preferred. Minimum of two years’ experience in the field of social services and working with children and adolescents with Intellectual Disabilities and/or Autism preferred About Woods Services Woods Services is a Pennsylvania and New Jersey-based non-profit population health network and advocacy organization that along with its six affiliate organizations provides life-long innovative, comprehensive and integrated health, education, housing, workforce, behavioral health and case management services to more than 22,000 children and adults in the intellectual and developmental disability, child welfare, behavioral and brain trauma public health sectors who have complex and intensive medical and behavioral healthcare needs. Our Langhorne campus is home to 550 residents, and provides outpatient and day programs. WCS offers a wide array of outpatient services to meet the unique needs of individuals from age 6 through adulthood including assessment, individual, group, and family therapies, medication coordination, nurse navigation, coordination of care, telepsychiatry, and telehealth. WCS serves most diagnoses, but has a targeted population of children and adults with Autism and Intellectual Disabilities.
