Operations Manager - Fairview Partners
**Overview** M Health Fairview has an immediate opening for an Operations Manager, Population Health and Value-Based Care to support the Fairview Partners (FVP) team. This is a 1.0 FTE (80 hours per two week pay period), salaried/exempt opening. **Responsibilities Job Description** The Operations Manager is responsible for oversight of the Fairview Partners Care Management Specialists (CMS) and supports the Care Coordination team with their day-to-day operations. The Operations Manager actively represents the care coordination department on the workflow design, process improvement, and ensures regulatory and health plan requirements are implemented as needed. The Operations Manager also provides project management support to the Fairview Partners leadership team and is an integral part of the management team. **Job Expectations:** Manages CMS staff + Provides oversite of the CMS team for all three Fairview Partners regions + Engages the team and tracks progress and goals through weekly Daily Engagement System (DES) huddles + Establishes performance and customer service standards for the CMS team + Meets regularly with CMS staff to discuss and promote employee development and to discuss performance and goals + Understands and follows human resources policies and practices Department Operations + Integral in the development, maintenance, and communication of care management processes + Is forward thinking and instrumental in evaluating standard work, looking for efficiencies and value-added changes, much of this through the DES work + Champions DHS policy changes and platforms to staff Oversees the Fairview Partners web-based enrollment and demographic platform. + Oversees Fairview Partners SharePoint data management, including department resources + Actively participates in committees to design and implement new processes that support departmental workflow + Works with IT and EPIC teams to learn and build processes that support efficient use of resources and increases productivity + Reviews health plan specific changes and DHS/CMS guidelines for regulatory compliance and integration + Serves as Customer Experience Pacesetter Provides operational support to the Program Manager of Population Health & Value Based Care + Actively participates in leadership discussions about department objectives + Carries out strategic initiatives as directed + Provides support in overall operational duties as assigned + Supports data integrity of system operations + Uses analytics and works with internal staff to incorporate initiatives focused on improving health plan quality standards Provides operational support to Director of Community Care Delivery + Develops performance improvement processes at the direction of department leadership and monitors outcomes + Provides project management for special projects and pilot programs. + Provides support for system and health plan quality initiatives in conjunction with department leadership and data analysts + Creates and maintains internal reports as requested by department leadership. + Takes the lead on organizing and maintaining service area mapping to assure even care coordination assignments and coverage areas + Participates in health plan operational meetings communicating needs and operational issues as they arise + Provides ongoing Sharepoint, One Drive, Teams and other technical education and training to the team as necessary Enrollment Operations + Coordinates monthly enrollment process with department leadership to ensure that all new clients are assigned to the care coordinators according to department and health plan contractual guidelines + Manages and delegates the monthly intake enrollment procedures with care coordination staff + Monitors and tracks monthly enrollment discrepancies for current members + Serves as the primary contact for health plans regarding enrollment and clinic changes + Provides support and troubleshoots ongoing problems, working with health plans to advocate for changes and resolutions that are mutually satisfactory + Oversees member updates to the Fairview Partners database as well as Epic + Monitors the nursing home Discharge to Home process from assignment to completion **Qualifications** **Required** **Education** Bachelor’s degree in a health care administration, business administration or health care related field **Experience** Minimum of 5 years of recent experience in health care field, operations, preferably customer service, management skills, and project management. Minimum of 6 years of leadership experience Knowledge of Microsoft Office, Word, Sharepoint, Power BI, database management, Adobe and EPIC. **Preferred** **Education** Bachelor’s degree in a health care administration, business administration or health care related field **Experience** Minimum of 5 years of recent experience in health care field, operations, preferably customer service, management skills, and project management.
