Oncology Patient Navigator

Summary
Assumes responsibility and accountability for the management of resources through interdisciplinary collaboration necessary and appropriate for the achievement of optimal patient care outcomes for oncology patients. Incorporates evidence-based research into practice across the continuum. Facilitates open communication and smooth transitions between multidisciplinary teams. Patient navigation in cancer care refers to individual assistance offered to patients, families and caregivers to help overcome barriers to care and facilitate timely access to quality medical and psychosocial care, from diagnosis through all phases of the cancer experience.
Education
Must have a Bachelor's degree in social work, psychology, sociology, or other field of social work.
Licensure/Certification
Current license preferred.
Experience
At least two years in health care or social service field.
Other Qualifications
Must be able to complete job duties autonomously. Demonstrates leadership skills and strong organizational skills. Ability to interpret and analyze medical and social information. Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion. Requires the ability to communicate and resolve conflict. Strong work ethic built on a foundation of proactivity and team work. Ability to navigate ambiguity with the aid of structured problem solving techniques.
