Oncology Nurse Navigator

What you'll do The Oncology Nurse Navigator will act as a primary point of contact for cancer patients after initial diagnosis and until patient has sufficient knowledge and resources to self-direct their care. The primary responsibility will be monitoring, coordinating care, and educating cancer patients and families as they move through the Bassett Healthcare system. The Oncology Nurse Navigator will contact cancer patients either in person or by telephone to establish a plan of care. It is the goal of the Oncology Nurse Navigator to assess the care and services required by cancer patients and make appropriate referrals. The Oncology Nurse Navigator will work in collaboration with social work and patient care coordinators for a smooth transition throughout the patient's cancer care. Collects and records relevant patient health data accurately, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation and co-worker feedback Assesses patients and reviews lab data to assure values within prescribed parameters prior to all treatment as documented in nursing notes and/or flow sheets as confirmed on random quarterly chart audits Analyzes assessment data to identify patient problems, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or supervisory observation Identified expected outcomes individualized to the patient, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation Implements and records interventions appropriate to diagnosis or patient problem, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation Provides appropriate teaching to 100% of patients and their family including written educational information re: disease, treatment regimen, supportive care, etc. as documented in the medical record Counsel’s patients for further exams and/or treatments, monitors symptom management either by telephone triage and/or at time of clinic visits as documented in the medical record as measured by random supervisory chart review and/or direct supervisory observation Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity as measured by no significant, substantiated patient or peer complaints received by the manager Assures that initial care processes are followed for patients who are treated for cancer care, including patient education by RN, prescriptions, and consent Acquires and maintains current knowledge and skills in his or her area of practice Demonstrates clinical leadership by serving as resource for the multidisciplinary team in the care of the cancer patient being followed in the navigation program Participates in 25 hours of educational activity related to job function annually and provides documented evidence of attendance or activity to supervisor (30 hours are required to exceed standards) Demonstrates clinical problem-solving ability through written exemplar, patient/family feedback, coworker report or direct managerial observation Demonstrates operational problem-solving ability through written exemplar, co-work report or direct managerial observation. Builds awareness of the Oncology Nurse Navigation program and the services that it provides to providers Responsible for day-to-day functioning of the Oncology Nurse Navigation Program Ensure safe practice standards are utilized Proactively develops and enhances processes with the goal of improving the clinical experience for referred patients and the referring physician Referrals to the navigation program should be responded to ASAP and not longer than 24 hours Communicate with the multidisciplinary team (physicians, nurses, social workers, patient care coordinators) as needed to complete assessment Work closely with physicians in all specialty areas to coordinate care for the patient, and communicates with all members of the healthcare team on behalf of the patient Gather pertinent information about the patient’s medical, psychosocial, functional, and financial status Provide necessary education and information of the treatment plan to the patients Educate the patients on their chemotherapy regimens Organize and facilitate multidisciplinary tumor conferences Participates in the Operational/Regulatory activities of the regional Cancer Program Designs and coordinate PI initiative demonstrating good organizational skills and follow-through as determined by direct managerial observation. Attends at least 80% of staff meetings as recorded on the attendance roster Attends at least 80% of all multidisciplinary tumor conferences Assists in the development, review and revision of the Regional Cancer Program Nursing Policies and Procedures as requested by supervisor pertaining to the navigation program Completes and delivers survivorship care plans according to ACS Commission on Cancer's standard 3.3 Collects and records relevant patient health information in the survivorship care plan according to disease process Follow up according to NCCN guidelines Works collaboratively with the physician to ensure information in each care plan is correct Successfully delivers survivorship care plans within the timeframe set by standard 3.3 of ASC Commission on Cancer Successfully tracks the number of delivered care plans in the electronic medical record Performs other duties as requested and observed by supervisor or manager What we need from you Education: Graduate from an Associate or Baccalaureate Degree program in Nursing, required All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN’s licensed on or before 12/19/17 are/remain exempt from this requirement.
