Job Summary:
Provide skilled occupational therapy to patients in their homes, improving independence in daily activities. Develop and implement treatment plans, assess progress, and educate patients and caregivers.
Key Responsibilities:
Evaluate patient needs and create customized therapy plans.Provide hands-on therapy to restore function and mobility.Educate patients and caregivers on exercises and adaptations.Document patient progress and collaborate with the care team.Ensure compliance with healthcare regulations and best practices.Qualifications:
Licensed Occupational Therapist (OTR/L)Experience in home health preferredStrong communication and assessment skillsMinimum Qualifications:
Graduate of an accredited occupational therapy program.Eligible for NBCOT exam; registered if required by state.Ability to work independently and maintain professional relationships.Commitment to ongoing skill development and policy adherence.At least 1 year of experience or 6-8 weeks with a preceptor.Home healthcare experience preferred.Valid driver’s license and reliable transportation.Adherence to mission, ethics, and compliance standards..
