POSITION PURPOSE AND SUMMARY     

The Occupational Therapist is a qualified professional who directs, supervises, evaluates and provides occupational  therapy services to patients as prescribed by the referring physician.  Develops plans that meet the functional, physical and educational needs of the patient. Observes response to treatment, documents and adjusts treatment for the quality provisions of care. Suggests areas of departmental and personal improvements related to therapy area. Attains required and procedural competencies.

EDUCATION/EXPERIENCE:

Graduate of an accredited Occupational Therapy curriculum with a minimum of a Bachelor of Science degree or graduation from an accredited college/university with a related major and completion of a certificate program in Occupational Therapy. National Certification required. State of Indiana Occupational Therapy License required, or eligible for state licensure within 180 days. Valid driver’s license and proof of auto insurance is required. Meets hiring criteria as defined in agency’s minimum standards. Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.
