A little more about our center:

Southview Manor (formerly Southview Care) is an inner-city skilled nursing facility (SNF) with 5 floors and an average census of 160+.  We have and multiple levels of care, including long term care and a large behavioral health (formerly indigent) population.

5 Star Quality measureThe average age of our patients is 35-55 and the most common behavioral health diagnoses are schizophrenia, dependency, bi-polar and major depressive disorders.Many are able to age in place, and about 25% return to the community.We have several programs encouraging engagement, but the most popular spot in our center is the game room. 

The National Association of Rehab Providers & Agencies (NARA) awarded Transitional Care Management their “Impact Award” for 2023 for our Life Skills Programming.  This center is using that very same “Life Skills Program” and changing lives every day.  Ask us about it during your interview – we’d be delighted to share our Social Services success stories.  It’s so much more than what your current Psych/Social team is doing now!

Nursing Home Administrator (LNHA)

Qualifications

Licensed Nursing Home Administrator (LNHA):  in other centers it may be called a Managing Director of Executive Director.

What we need from you:

IL LNHA license 3+ years of experience as an LNHA in a large skilled nursing facility (experience behavioral health is a plus)Extensive knowledge & experience with state and federal care regulationsYou understand ROI and know your buildings metrics inside and outIn your last position you demonstrated the ability to work with minimal supervision & handle multiple priorities at once

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
