UM Upper Chesapeake Health (UM UCH), a member of the University of Maryland Medical System (UMMS), has a Full-time employed position available for a Nurse Practitioner Program Coordinator to join their reputable, well-established transitional care clinic (TCC) located on the UM Upper Chesapeake Medical Center campus in Bel Air, MD.
Nurse Practitioner Job Details:
Position is responsible for diagnosing and treating patients independently and in collaboration with physicians and colleagues.This NP will care for complex patients with multiple comorbidities and chronic conditions who are transitioning from the acute hospital setting to the home. The NP will also develop and evaluate programs related to population health care initiatives to identify opportunities for clinical interventions to reduce admissions.Provides continuity of patient care by facilitating and coordinating communication between the health care team, patient, and the patients’ family. Coordinates care to ensure quality, effective care and patient safety.This NP will work on a collegial, multidisciplinary team comprised of physicians, nurse practitioners, nurses, social workers, pharmacists and more.The ideal candidate will have a strong interest in population health and share in the goal of the program, which is to decrease patient readmittance rates.Monday through Friday work schedule. QualificationsEducation & Training: Master of Science degree with completion of a Nurse Practitioner program required. National certification in the area specialty and licensed by the Maryland Board of Nursing. NPI number, DEA and CDS required. Doctorate of Nursing Practice preferred. BLS required; ACLS preferred.
Work Orientation & Experience: Minimum of three (3) years of acute care or related experience required.
