Nurse Practitioner - Palliative Medicine
T he Palliative Medicine Service Nurse Practitioner works as a member of a multi-disciplinary clinical team. Responsibilities include working directly with patients and their families/support network and collaborating with a patient’s attending physician, specialty healthcare providers, nursing staff, and ancillary staff to identify and facilitate goals of care. This involves independent consultation and patient assessment, good communication, and provision of education and supportive resources when needed. These consultation services may be requested to be provided during an initial episode of hospital admission or at recurrent hospital admissions for serious or progressive illness, and under standard conditions or in crisis situations. The Palliative Medicine Service NP may be called upon to have advance care planning conversations with patients and/or their decision makers, including completion of any associated MOLST or HCP forms, or to provide assessment and recommendations regarding complex pain or other symptom management. In the case of the latter, the NP will develop treatment recommendations in collaboration with the Palliative Medicine Service Physician. In addition, the Palliative service NP will not infrequently need to determine a patient’s capacity to make healthcare decisions for themselves, facilitate family meetings, facilitate referrals to Hospice and Francis House, and implement comfort care orders. Job Requirements: Required: Currently licensed as a Registered Nurse in New York State. Currently licensed as a Nurse Practitioner in Adult, Family Practice or Specialty area in New York State. Graduate of an accredited Nurse Practitioner program. Minimum of two (2) years’ clinical experience as a nurse practitioner preferably in medical/surgical inpatient care, critical care, pain management, oncology, or Hospice. Basic Life Support (BLS) certification required at the time of hire or within two (2) weeks after hire. Advanced Cardiac Life Support (ACLS) required at the time of hire or within two (2) weeks after hire. Maintenance of AHA Stroke Certification is expected. The ideal candidate will have good communication skills, desire to learn, ability and willingness to work collaboratively with other healthcare providers and as part of a multi-disciplinary team. Advanced Practice Certification in Hospice and Palliative Medicine (HPM) is preferred, but not required at time of initial employment. An agreement to work towards attainment of Advanced Practice Certification in the subspecialty of HPM is required. Pay range: $47.00 - $66.54
