This advanced practice provider will be focused on the well-being of Onsite employees and their beneficiaries, at Hoosier School Benefit Trust, South.

Community Health Network has an excellent opportunity for an experienced, family medicine certified advanced practice provider to practice in an Employer Health Onsite primary care role. This opportunity is at Hoosier School Benefit Trust, South located at 6925 South Harding, Indianapolis, IN 46217.

Job Duties

Routine check-ups and preventive care Diagnosis and treatment of illness or injury Managing chronic or complex health conditions Medication management Referral to specialists for more serious conditions

 

Experience

Educational background consistent with requirements for licensure and certification with the State of Indiana Minimum two years of experience as an advanced practice provider is required.
