Nurse Practitioner
Full-Time Nurse Practitioner Reports To: Chief of Internal Medicine, Queens Queens/Manhattan, New York United States Responsibilities : + Involve with direct patient physical assessment, education, and health planning + Maintain continuity of care by regular patient follow-up + Assist in the coordination of services with other disciplines and patient's insurance plans + Advocate for patient in arrangement of medically necessary services + Communicate with community's resources to ascertain adequate assistance for patients in need + Refer patients to specialty services when necessary + Consult with collaborating physician and other members of the health care team to establish plan of care + Participate in quality assurance process to assure optimal care delivery + Provide comprehensive case management services that include emphasis on health promotion and disease prevention + Monitor patient outcome in accordance to the practice protocols + Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning + Act a resource person for the nursing staff + Conduct education in service on practice issues + Perform other duties as assigned Minimum Qualifications : + Graduated from approved Nurse Practitioner Program + Licensed as Nurse Practitioner in New York State + Basic CPR (BCLS) certification required + At least 1 to 2 years of clinical experience as a primary care Nurse Practitioner + Board/Specialty Certification preferred + Knowledge of a second language (such as Mandarin, Cantonese, Korean, Spanish) preferred Compensation and Benefits : + Salary: $105,000.00 - 115,000.00 per year and p roductivity-based incentive pay + Medical insurance + Employer-paid dental, disability and life insurance + Employer-paid malpractice coverage through FTCA + 403(b) retirement savings plan + Flexible spending accounts for transit, parking, health and dependent care + Paid time off (6 weeks) + Continuing medical education time and allowance + Employee Assistance Program + National Health Services Corps loan repayment program available, and + The Health Center is qualified for Public Service Loan Forgiveness (PSLF) program
