Facility:Dayton Children's - Main CampusDepartment:Pulmonary ClinicSchedule:Full timeHours:40Job Details:In congruence with the holistic care provided by the Division of Nursing and Division of Pulmonary Medicine, the CF Care Center Coordinator (CF CCC) works with all members on the CF Care Team to ensure that the CF Program runs efficiently. The CF CCC is required by the CF Foundation (CFF) in all accredited CF Centers. The CF CCC has oversight and input in CF Center organization and coordination of the CF Team. The person in this role is required to exercise leadership, organization, communication, decision-making and collaboration skills. The CF CCC helps coordinate and carry out medical care plans, facilitates communication between patients/families and other members on the CF Care Team. The CF CCC alerts the CF Care Team about psychological, social, financial concerns, and provides health information or directs patients/families to resources to help manage disease, and help educate patient, family, and other health care professionals. Transition of care should occur between ages of 18-21 years. All centers must provide CF care for adults as outlined by the CFF which also includes the multi-disciplinary care Team Members, i.e.; Director, Associate Director, coordinator, dietitian, respiratory therapist and social worker. The CF CCC will work closely with the multidisciplinary CF Team which includes, Physicians, Respiratory Therapists, Nursing, Lab, Dietitian, and Social Services along with external stakeholders, providers and institutions to facilitate continuity of care. There is potential to collaborate with all other hospital departments as well.
