Nurse Navigator - Full Time - Day Shift

Position Purpose:

The Nurse Navigator is the strategic point of contact for cancer patients and their families. The Nurse Navigator’s primary function is assisting patients and families through the healthcare system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes with the goal of achieving optimal patient outcomes, and tracking interventions and outcomes to achieve optimal patient outcomes. The Nurse Navigator serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance accessing clinical and supportive care services while navigating patients through the diagnostic evaluation process. The Nurse Navigator assists patients in understanding their diagnosis, treatment options and resources. 

What you will Do:

Initial Referral – Receives the initial referral from the referring physician and serves as the program representative to the referral base.  Serves as the point person for incoming calls and facilitates additional testing. 

Diagnostic Work-Up—Use the National Comprehensive Cancer Network (NCCN) Guidelines as a resource when working with referring providers, surgeons, and/or oncologists to facilitate a rapid staging workup of newly diagnosed cancer cases.  

Physician Consults— a cancer diagnosis is confirmed, the Nurse Navigator initiates the process for the patient to receive appropriate consults on time and provides the patient and family with an overview of consults/clinic visits. 

Patient/Family Education – performs direct teaching required for patient/family success, approaches teaching at every point of interaction, refers patient/family to other education resources, and serves as the point person for all questions related to overall care experience.  Recognizes patients and family's diversity and/or cultural needs and uses appropriate educational methods.  Physically present at consults when possible and utilizes effective communication skills to convey information to/between patient and the multidisciplinary team members.

Psychosocial Care – Assesses the psychological response of patient and family to determine the need for referrals to appropriate support systems in dealing with the stress.  May serve as a direct resource to patients and families to discuss personal concerns and issues. Provide referrals to Oncology Social Worker as appropriate.

Hours/Schedule:

FTE 1; Full Time; Day Shift

Minimum Qualifications:

Current RN license in the state of Iowa.

BSN must be obtained with in 5 years of hire 

Must complete and pass required Oncology Nursing Society (ONS) courses upon hire. Including, but limited to the Cancer Basics course and the Chemotherapy Biotherapy: Fundamentals course.

Highly recommended to obtain an Oncology Nursing Certificate (OCN) within 2 years of employment.

Minimum of three years of clinical experience in oncology-related modality.
