Nurse Manager - Home Care
*POSITION SUMMARY* To be fully engaged in providing No Harm/ Quality care, Customer Experience, and Stewardship through the coordination, implementation, and optimization of appropriate, patient focused care while ensuring clinical, customer/patient satisfaction, regulatory and financial goals are being met. The manager is responsible for supervision of supervisory and clinical staff, the intake department, and clinical liaisons, while supporting the IDN in a collaborative relationship with our hospital leaders, medical group and community partners. *PRIMARY ACCOUNTABILITIES* Lead daily clinical operations to ensure quality patient care, appropriate utilization, regulatory requirements, and customer/patient satisfaction. Demonstrate and promote strategic thinking and drive problem-solving by empowering and supporting clinicians to assume responsibility and accountability within their scope of practice. Maintain and disseminate best practice clinical information obtained from across Health First and from external resources. Provide performance tools and a system of accountability by proactively monitoring leading indicators for patient safety, comparing performance outcomes against top decile benchmarks, and ensuring action planning to continuously improve patient outcomes and customer experience. Recruits, interviews, promotes and disciplines while providing ongoing coaching, competencies and staff development. Oversee and manage the development of appropriate policies and procedures to ensure compliance with the necessary regulatory agencies including AHCA, The Joint Commission, etc. Monitor and ensure compliances with resource allocation. Direct quality and efficient care coordination to ensure an optimal member/patient experience and maximize customer satisfaction. Job Description Lead business development initiatives for strategies to build referral volume by managing hospital and non-hospital liaison team to achieve financial benchmarks. Manage the clinical intake referral team by ensuring the safety and appropriateness of patient acceptance and facilitate timely initiation of care with proper disciplinary team members. Serves as an IDN partner to by mediating and assisting with complex problem-solving, conflict resolution, and prioritization utilizing continuous improvement principles. *LEADER ACCOUNTABILITIES * 1. Define and communicate a clear, compelling vision for the team that effectively ties into the mission and vision of Health First, and inspirationally lead the team to achieve that vision. 2. Provide leadership, motivation, coaching, feedback, and support to foster and strengthen growth and development of an effective, high performing team. 3. Contribute to and support the strategic direction, and demonstrate financial acumen, for areas of responsibility and organization. *MINIMUM QUALIFICATIONS* • Education: Graduate of an accredited school of Professional Nursing • Work Experience: Minimum of 3 years Healthcare Leadership experience • Licensure: State of Florida License in a clinical concentration • Certification: AHA/BLS Healthcare Provider Completion Card within 30 days of hire • Skills/Knowledge/Abilities: Five (5) years clinical management or leadership role ** *PREFERRED QUALIFICATIONS* • Education: BSN required • Work Experience: Homecare leadership experience preferred • Certification: AHA/BLS Healthcare Provider Completion Card • Skills/Knowledge/Abilities: OASIS documentation, CMS Conditions of Participations, ACHA Homecare Standards, TJC Standards PHYSICAL REQUIREMENTS (ADA, Travel, Environment, Noise) Majority of time involves sitting or standing; occasional walking, bending, and stooping Long periods of computer time or at workstation Light work that may include lifting or moving objects up to 20 pounds with or without assistance. May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise Communicating with others to exchange information.
