Job Description

We are seeking a dedicated Nurse Case Manager to join our team within a surgical setting in the Orthopedics department. This role is perfect for nurses looking to step back from heavy clinical roles and focus on care coordination and team leadership.

ResponsibilitiesCoordinate care for a select caseload of patients from triage through care coordination during management and safe transfer to home.Proactively communicate with payors to ensure approval of treatment plans and required resources.Maintain knowledge of and respond to clinical system and fiscal data related to specific patient populations.Collaborate with the multi-disciplinary team, patients, and patient families to ensure effective and efficient transition planning.Coordinate transition to post-acute settings including nursing facilities, acute inpatient rehab, and local shelters.Manage patients based on departmental procedures while maintaining clinical safety and quality.Enter and maintain patient information in all departmental systems as per operational guidelines.Essential SkillsActive RN License2 years RN experienceBLS CertificationCurrent in-patient Case Management experienceExperience with post-acute facility referral platforms such as NaviHealth or AllScriptsAdditional Skills & QualificationsExperience with EPICCase Management CertificationProficiency with EMR and nursing documentation softwareStrong leadership and organizational skillsTech-savvy with excellent medical knowledgeWork Environment

This position involves working within a surgical setting in the Orthopedics department. The role consists of five 8-hour shifts from 8:30 am to 5 pm, with no weekends, holidays, or on-call duties. The nurse case manager will primarily work behind a computer, managing the nursing team and ensuring streamlined and efficient patient care. This position offers a more comfortable role within the hospital, with opportunities for leadership career growth.

Pay and Benefits

The pay range for this position is $35.00 - $45.00/hr.
