Join our dynamic Healthcare Home team as a Nurse Care Manager and leverage your deep understanding of chronic disease processes in children and adults. Empower patients with health education, consult with community support specialists, manage hospital admissions and discharge processes, and guide patients through diagnoses, treatments, and medications.

Key Responsibilities:
• Develop wellness and preventive initiatives.
• Spearhead health education classes, tapping into profound insights on complex medical conditions.
• Contribute to the preliminary treatment scheme creation for all Healthcare Home participants.
• Team up with professionals to design a targeted treatment strategy comprising healthcare objectives for individuals afflicted by dual chronic diseases.
• Advise Community Support Specialists on patient-identified health complications.
• Streamline hospital and medical facility admission and discharge proceedings for Healthcare Home beneficiaries.
• Coach patients on complex health conditions, discussing medical diagnoses, therapies, and prescriptions.
• Execute DMH Net Health technology projects and endeavors, such as CyberAccess and metabolic screening.
• Track and relay performance metrics and triumphs using refined information systems engineered for nurses.
Education and/or Experience Qualifications:
• Active RN or LPN license
• Experience in direct patient care recommended
Additional Qualifications:
• Deep understanding of acute and persistent medical conditions affecting children and adults
• Outstanding written and spoken communication prowess
• Agility in accepting and applying novel, creative systems
• Decision-making capabilities founded on critical thinking and nursing procedure
