Responsibilities

What will you be doing?

Provide case management services (information, plan referral, and linkages) as neededDevelop and maintain clinical records and program documentation (assessments, treatment plans, progress notes, termination summaries, etc.)Coordinate and advocate for services with other professionals and paraprofessionalsConduct community outreach as neededArrange for transportation of clients to appointments, community agencies, etc., as neededWork with the client's family to address issues related to the client's service planMonitor the appropriateness of services to the clientComplete billing-related documentation for the Medicaid billing department ensuring billing documentation is accurate and completeParticipate in clinical review of clients for services and enhanced placement when appropriateAttend staffing on behalf of the client to ensure the needs of the client are being addressed fully (schools, clinical, etc.)Complete documentation related to authorization for Targeted Case Manager (TCM) servicesThis is a part-time to full-time transition position Qualifications

Are you a good fit?

We are seeking a highly skilled and motivated individual who meets the following criteria:

Candidate has strong organizational skillsPossess exceptional leadership, interpersonal, and communication skills that allow you to work well with diverse teams, clients, and familiesPossess some knowledge of the Wraparound System of CareMust be Bilingual in English and Spanish

Must Have:

Bachelor's degree with a major in a human services-related fieldA minimum of one year full-time or equivalent experience working with severely emotionally disturbed childrenFlorida Driver’s License within 30 days from hire

           OR

Bachelor's degree in a non-human services-related fieldThree years of full-time or equivalent experience working with severely emotionally disturbed childrenSuccessful completion of AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services

*Some travel is required for this position*
