Join our St. Cloud Hospital Care Management Department and support the Neuroscience and Spine Unit as a full time medical social worker. 

Our Socials Workers at the St. Cloud Hospital serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.

We will provide supervision for new grads in-house or will reimburse up to $175/month for external supervision.

Schedule Full-time | 64 hours every two weeks | Onsite Day shift | 8-hour shifts | Between the hours of 7:30am-4:00pm | Monday through Friday Pay & Benefits  Entry level pay $27.95/hr; however, we pay more for experience $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates! Qualifications Current Licensed Social Worker (LSW) in Minnesota required.  Bachelor’s degree in social work or Sociology.  At least two years of experience as a Social Worker, preferably in a healthcare setting. American Case Management Association (ACMA) Case Manager-SW Certification required within five years.

Hiring bonus requirements: To be eligible for the hiring bonus you must be an external candidate who is hired into an eligible full time or part time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.

Relocation assistance requirements: Must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. Must currently reside more than 50 miles away from St. Cloud and relocate to within 30 miles of it.
