Shift: Monday-Friday\/8:00am-4:30pm\n
Primary Purpose:\n\nTo coordinate delivery of services to managed care and Medicare residents in collaboration with the facility's team members.\nAssist the Director of Clinical Services with ensuring that all documentation in the facility meets federal and state, certification guidelines. \nCoordinate the entire RAI process assuring the accuracy, timeliness, and completeness of the MDS, RAPS, and Interdisciplinary Care Plan.\nMonitor and document the cost effectiveness of treatment provided, facilitates and coordinates the admission and discharge process, serves as the resident and family advocate, and acts as a liaison to insurance and medical management professionals.\n\n
Major Responsibilities:\n\nComplete accurate and timely MDSs, CAAs, and plans of care in accordance with the RAI guidelines for all residents in the facility.\nProvide resident status updates as needed to appropriate personnel.\nComply with all of the duties and responsibilities noted for the MDS position.\nPerform other duties as assigned.\n\n
Specific Requirements:\n\nMust be able to read, write, speak, and understand the English language.\nMust possess excellent interpersonal and communication skills.\nMust demonstrate the knowledge and skills necessary to prove care appropriate to the age-related needs of the residents served.\nMust be a supportive team member, contribute to, and be an example of teamwork and team concept.\nMust be knowledgeable of nursing and medical practices and procedures, as well as, laws, regulations, and guidelines that pertain to nursing care facilities.\n\n
Working Conditions:\n\nWorks in office area(s), as well as, throughout the nursing service area.\nMoves intermittently during work hours.\nIs subject to frequent interruptions.\nIs involved with residents, personnel, visitors, government agencies\/personnel, etc., under all conditions and circumstances.\nIs subject to hostile and emotionally upset residents, family members, personnel, and visitors.\nIs subject to call back during emergency conditions (severe weather, evacuation, post-disaster, etc.)\nAttends and participates in continuing educational programs.\nIs subject to exposure to infectious waste, diseases, conditions, etc., including TB, AIDS, and Hepatitis B viruses.\n\n

Minimum Qualifications:\n\nMust possess a current, unencumbered, active state license to practice as an (RN or LPN).\nExperience in Skilled Nursing\/Rehabilitation facilities, required.\nSix (6) months of experience as a MDS Coordinator, required.\nRAC-CT or RNAC, preferred.\n\n
