This position includes but not limited to the following duties; helping with admissions and discharges, organize Med A meetings which include communication with physicians, families and our therapy department, oversees resident care plans, familiar with ICD - 10 coding, managing assessments, transmitting and verifying MDS assessment and assisting with floor needs. We are looking for an individual who is organized, and completes items in a timely manner. This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans. Completes the assigned portions of the MDS for Nursing. Signs the MDS forms to signify completion of the process. Maintains residents’ self-esteem, privacy and confidentiality of personal information. Assists to maintain safe, pleasant, orderly resident living areas. Completes nursing portion of MDS and reviews for completion and validity of entire process. Queries other members for accuracy of MDS; and provides final quality check prior to MDS transmission. Receives and shares information. Observes and reports residents’ conditions. Communicates with physicians using center/campus systems. Directs and assists with co-worker, resident, family/significant other education. Provides support and recognition to co-workers related to completion of the RAI / MDS process. Uses the EMR accurately. When functioning on the floor, the Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care to patients. The RN demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. The RN cares for residents in all phases of preventative care, health maintenance, diagnosis, and treatment. The RN is responsible for the coordination of care, resident assessment, resident education, and various other nursing interventions. The RN may serve as charge nurse and delegate appropriate tasks to licensed and unlicensed assistive personnel that are within the job descriptions, skills, resident care needs, and the competence level of the delegate. As charge, the RN provides adequate assistance and support to delegates to ensure safe, reliable administration of resident care and resolution of care and service concerns. The RN functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct. **Qualifications** Join our Nursing Management team that cares for fifty residents. We’re looking for an RN with a minimum of 1 year of experience working as an RN in Long Term care setting with knowledge of MDS, Medicare and care planning processes. To be successful, you will need good communication skills, great attention to details, and meet deadlines, set timelines, and be able to collect information from a variety of sources.
