Manage assigned Case Management and Health Navigation programs and staff within the Client Services Department, in accordance with policies and regulations. The position will assist the Director in managing all aspects of the assigned program and department activities and staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

Monitor, evaluate and implement program quality assurance, improvement and clinical integrity through chart reviews, surveys, policy and procedure revision and other applicable meansProvide input, oversight and monitoring to program activities, including development of collaborating agencies and sites for intervention/prevention activitiesMonitors and support clinical competencies through accepted discipline supervision standards.Supervise daily activities of assigned staff and implementation of evidenced based modelsMaintains appropriate staffing patterns with qualified personnel, supervision and training.Assess, develop, locate and track training needs for program staff.Monitoring program productivity, outcomes and service delivery goalsAssist in the development, implementation, monitoring and evaluation of program services, initiatives, procedures and standards.Attend and Facilitate community, program meetings, staff meetings and trainings as determined by Director Maintains databases, data tracking systems and reports in accordance with agency and program, licensing and accreditation standardsAddress and document client and provider concerns prior to the grievance procedure and Director Involvement.Represent the agency at community forums and provider consortiaKNOWLEDGE, SKILLS, AND ABILITIES

Advanced knowledge of HIV disease, intervention and prevention.Advance Knowledge of Case Management skillsKnowledge of Substance Abuse/Mental Health intervention and prevention.Effective verbal and written communication skills Ability to establish and maintain effective working relationships with employees, collaborating agencies and the publicAbility to demonstrate and model culturally competent behaviorModerate to advanced computer skills required: Microsoft Office, Multiple Databases, Scheduling ProgramsBasic math skills required: addition, subtraction, multiplication, and divisionAbility to work independently with minimal directionBasic office equipment operation skillsAbility to work under pressure and to assess changing priorities and competing deadlines is essentialFlexibility and ability to cope with several tasks simultaneouslyMust be physically and mentally fit to respond to emergency situations.EDUCATION AND WORK EXPERIENCE

 

(3) years related field in health services or Bachelor’s degree in the Behavioral Health SciencesOne year of program management and/or staff supervision

 CERTIFICATES AND LICENSES

HIV 500Valid Florida Drivers License and ongoing proof of insurance requiredMust have reliable transportation for work related activities

Must pass all drug and background screening requirements, including a level 2 federal background and motor vehicle screening
