Job Summary:
This position provides regional leadership, supervision and clinical oversight of Hospice and Home-Based Palliative Care Social Work, Pastoral Care/Chaplain, Bereavement staff and the Volunteer Program. Provides clinical guidance for Home Health Social Workers. This position collaborates with all hospice and palliative care Interdisciplinary teams to ensure quality and timeliness of these services that meet all state, federal and Joint Commission regulations and standards.
Essential Responsibilities:
+ Directly and through subordinates, plans, organizes, directs, and controls the operation of assigned areas (e.g., general administration, patient care, professional services and/or a combination of these and other services). Contributes toward the overall development of programs and services to provide for the needs of members, physicians and staff. Provides strategic leadership to management to ensure equitable distribution of resources and delivery of high quality, cost-effective healthcare and efficient management of staff in accordance with goals, objectives, policies, and applicable regulatory agency rules and regulations. Co-leads unit-based teams (UBTs). Ensures the UBTs continuously focus on identifying and implementing ways to meet/exceed quality, service, and access targets and implement regional initiatives. Collaborates with other departments to ensure smooth transitions and coordination of care.
+ Responsible for the day-to-day business operations by facilitating established department goals, implementation and monitoring of service, quality, utilization standards and regulatory requirements as well as appropriate policies, standards, practices, legal and regulatory requirements and government/accrediting agencies. Evaluates, plans and organizes service and/or care delivered by staff. Responsibility for adhering to the department budget, staffing, expenditures and payroll. Responsible for hiring, performance management and terminations as well as mentoring and development of staff.
+ Provide clinical oversight and development of MSW staff to ensure staff are at capacity and have resources available to ensure growth and development. Provides home visit supervision and feedback. Oversees orientation of new MSW, Chaplain and Bereavement employees. Assures opportunity for case review and continuing education. In collaboration with CCS leadership and labor develop curriculum, training and mentoring as indicated.
+ Define and communicate professional practice with and for CCS Social Workers, Bereavement staff, Pastoral Care/Chaplains and their Team and Program Manager.
+ Ensures that staff utilizes Kaiser Permanente Continuing Care Services departmental and community resources in a cost-effective manner. Consults with other interdisciplinary team members and shares work, social expertise and perspective.
+ Actively participates in quality management and quality improvement activities within the program including, but not limited to, patient rounding, clinical record review, occurrence reporting, focused studies, process and outcome measurement and continuous quality improvement work.
+ Participates in weekend and holiday supervisor rotation.
Basic Qualifications:
Experience
+ Minimum three (3) years of general management experience in a complex, multi-faceted healthcare setting and in working with a large physician group practice managing through influence and collaboration, including supervisory responsibilities.
+ Minimum three (3) years of experience in financial management, budget development, program development, quality improvement and project management.
+ Minimum one (1) year medical social work experience.
+ Minimum two (2) years of field experience managing complex cases within a multidisciplinary team in a home health, or hospice-palliative care setting.
Education
+ Masters degree in social work from an accredited school.
License, Certification, Registration
+ This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.
+ Driver's License (Washington) within 1 months of hire OR Driver's License (Oregon) within 1 months of hire
+ Licensed Clinical Social Worker (Oregon) within 6 months of hire
+ Licensed Independent Clinical Social Worker (Washington) within 6 months of hire
+ Driver's License (from any state) required at hire
+ Basic Life Support required at hire
+ Advanced Certified Hospice and Palliative Social Worker within 12 months of hire
Additional Requirements:
+ Previous experience with program planning, implementation and evaluation.
+ Thorough understanding of Social Work practice across a variety of medical settings and specialties.
+ Thorough knowledge of Hospice Conditions of Participation.
+ Demonstrates customer-focused service skills.
+ High level communication skills which allow for collaborative and timely interactions with clinicians, managers, staff, members and community partners.
+ How to perform a mental status exam.
+ Keyboarding or typing skills.
+ Previous work in a Hospice setting.
+ Previous Bereavement experience.
+ Experience with interdisciplinary team function.
+ Working knowledge of the managed care industry.
+ Previous experience working in Union/Partnership atmosphere.
+ Knowledge of Medicare regulations/requirements.
+ Excellent time management, problem solving and customer service skills.
+ Excellent written and verbal communication skills.

Preferred Qualifications:
+ Minimum four (4) years of experience as a medical social worker in a hospice setting.
