The Care at Home LPN monitors and navigates patients and families across the continuum of the medically home phases of care. In this role, you will perform assessments, monitoring, planning and interventions for acute care patient populations in the home while using in-home technology and protocols in collaboration with other members of the health care team. **Please not** **e that this is not a work from home position, and you will be required to work on-site in Vero Beach, FL providing virtual nursing care to patients from a command center.** **A caregiver in this position works three 12-hour shifts from 7:00PM - 7:30AM with rotating weekends and holidays.** Minimum qualifications for a caregiver in this role are: + Graduate of Accredited LPN program. + Florida Licensed Practical Nurse (LPN) license. + Basic Life Support (BLS) Certification Preferred qualifications for the ideal caregiver are: + Three years of LPN experience in an inpatient hospital or long term care setting + Demonstrates a strong work ethic. + Quick learner. + Thrives in a team environment. + Provides care with compassion, positivity, and patience. + Has excellent interpersonal and communication skills.
