Licensed Social Worker

Skills: Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.

Job Responsibilities: Resident/Family/Physician Intervention Psychosocial Services Vision, Hearing, Dental Services Assessments & Care Planning Discharge Planning Requirements: Prefer 2 Years Social Work experience in a SNF or Hospital 4 year Degree Social Worker License in the State of Ohio
