Job Summary The Licensed Practical Nurse provides normalized, therapeutic, and preventative nursing care to individuals in a given assignment at Woods Services within the scope of the Pennsylvania Practical Nurse Law and in accordance with licensing standards and Woods Services Policies and Procedures. Maintains communication with Campus Nurses and Nursing Management. Assessment Assess assigned individuals' health status on admission and discharge from Woods or the hospital/ER. Assess individuals when medical concerns are brought to their attention by Woods staff including MTS, MCS, on incident and seizure reports, etc. Assess assigned individuals who appear in the campus report, pertinent to needs as addressed in the report. Refers to individuals to charge nurses/physicians/providers when further assessment is indicated. Differentiates between the need for emergency care or the need for physician/provider follow-up making arrangements in conjunction with nursing management, charge nurse, medical scheduler, or the Medical Center at Woods staff. Ensures monthly documentation of weights/heights is completed in the assigned individual’s electronic health record (EHR). Monitor for any changes and communicate concerns to dietitians/physicians/providers. Assists physicians/providers with exams/assessments in the residence, day program and medical clinics as necessary. Attends and actively participates in psychotropic medication reviews and actively participate. Complete AIMS screening according to psychotropic monitoring protocols. Completes Lifetime Medical Assessments 45 days prior to the individual’s team meeting. Medication Administration Prepares and administers medications as ordered by Woods' providers in accordance with Woods' Medication Procedures. Evaluates the therapeutic effectiveness of medication with pertinent data and report any observed side effects or relevant laboratory values to Woods' providers. Reviews the assigned individual’s electronic medication administration record (EMAR) daily during the scheduled shift and reviews/approves new and discontinued provider orders before the shift is completed. Assist with observing medication administration of MTS and MCS as needed. Assists with observing medication administration of MTS and MCS as needed. Administers immunizations, treatments, and screenings as ordered by the providers. Documents all relevant information related to medication administration. Reviews the assigned individual’s EMAR after each medication administration and ensure required documentation is complete: meds were administered and signed out, treatments completed, and vital signs obtained when necessary. Assists individuals with the self-administration of medication goals when necessary and as indicated by the SAM assessment and core team goals. Assists with instruction and education for MTS/MCS on any treatment in which they need to demonstrate competence and document on Training Form. Assists MTS/MCS with any discrepancies between EMAR and labels on medication. Assist MTS/MCS when a supply of medication/treatment is needed. Collaborates with MTS/MCS and ensure medications/treatment and medical supplies are re-ordered in a timely manner. Communication/Documentation Responsibilities Documents assessments, health needs, health concerns and medical interventions in the individual’s EHR and the electronic campus report daily. Documents status of an individual’s medical appointment in their EHR. Prepares and complete consult or transfer forms for on and off campus appointments including hospital and emergency room visits. Responsible for division of paperwork with other unit nurses/MTS on all shifts. Responsible for checking messages and communication via computer/voicemail at least twice/shift. Relays pertinent health concerns (i.e. seizures, elevated temperatures, unusual behavior, rashes, etc.) to the charge nurse, nursing management, providers, or the Medical Center at Woods staff immediately. Coordinates medical appointments with the individual’s core team and notify family/guardian. Actively participates in the Interdisciplinary Team process, as necessary. Coordinates with the phlebotomist to ensure lab work is completed. Notifies family/guardians/agencies/Residential Manager/Day Program Manager of health issues as necessary. Contact family/guardians/agencies for permission to proceed for non-routine testing. Consult with providers/family if insurance will not pay for specific tests/procedures. Maintains professional conduct during interactions with all Woods’ employees, residents and families. Maintains courteous interaction and excellent communication skills. Continuing Education Participates in continuing education relevant to the area of nursing. Obtains a minimum of 40 hours of in-service training per year. Obtains CPR certification yearly, First Aid certification every three years, and other mandatory in-services. Conducts in-service as a resource person regarding health issues for residential staff/MTS/MCS and documents on training forms. Provides health teaching to individuals at Woods as necessary. Attends nurses' meetings/in services and serves on committees when requested. Responsible for reading minutes from meetings and knowing the information contained within. Responsible for obtaining in-service/knowledge of new equipment/procedures prior to operating new equipment/performing procedures. Other Responsibilities Keeps nursing stations neat and orderly, and orders supplies as needed. Covers units other than scheduled assignments when necessary for campus-wide coverage. Rotate coverage in school programs. Carry radio for school and campus coverage as assigned. Assures that an adequate supply of medications is available at all times. Assists with emergencies/fire drills in units and evacuating residents as necessary. Maintains medical equipment in units, assesses the functioning of emergency equipment monthly and after use, and reports any malfunctioning equipment immediately to nursing administration. Coordinates assessments/medication issues, etc and make rounds in residencies throughout assigned shift. Assists in the orientation of new nursing employees. Be competent in Wood’s EHRs and EMAR. Maintains HR requirements for personal physical and Mantoux testing. Current licensure in good standing as Practical Nurse in PA. Current driver’s license and reliable transportation Current CPR certification or completion of the course within six months Must be able to lift a minimum of 50 pounds.
