Summary This position is a member of the Primary Care PACT Teams. The Licensed Practical Nurse (LPN) functions as a member of the nursing and treatment teams in planning and implementing a full range of practical nursing care for veterans. The LPN provides complex age-specific nursing care for adult and geriatric inpatients or outpatients with physical, mental, behavioral, or psychosocial needs, according to assignment and training. LPNs at this grade level perform all duties expected of LPNs at the level, however, they are expected to be technically proficient in initiating, performing, and completing assigned duties in providing care to variable patient populations, requiring minimal guidance and direction. The LPN nurse is characterized by the employee's: a. Demonstrated ability to vary the order and sequence of assigned procedures based on the patient's condition, to use judgment in selecting the most appropriate application of procedures and guidelines, and to accurately report and record significant patient information. Completed work should need only a general review by RN or provider for appropriateness and conformity with established policies and procedures. b. Demonstrated ability to observe, identify, and respond to the patient's needs for nursing, medication, and equipment-assisted care, and to recognize deviations from normal. In modifying care plans, the LPN considers physical, emotional, and social factors. c. Practical knowledge of human behavior, motivations, and reactions to situations. Demonstrated ability to use this knowledge in working effectively with patients, families, and other staff members. Patient care activities, as appropriate to assignment, (includes, but is not limited to): a) Provides for the patient's privacy and safety. b) Assists with data collection for outpatients and new admissions. Observes, reports and documents signs and symptoms of patient's general condition. Actively contributes to nursing and/or interdisciplinary team conferences, exchanging information about patient needs and condition, and assists in the nursing management of patients with complex problems. Provides input into the development, implementation, and evaluation of the Nursing Care Plan and/or the Interdisciplinary Plan of Care. Preferred Experience: The individual selected for this position works under the direction of the Registered Nurse (RN) and within identified Outpatient clinic teams. Responsible for the treatment, observation, and care of the Veteran within the scope of LPN functions. Assists in the overall clinic management, functions and operations, facilitation of patient flow, through the check in and checkout process. Provides patient education and/or instructions and appropriate documentation. Works with multiple specialty clinics. Participates in job/systems redesign activities and other activities that enhance patient care and clinic operations. Maintains patient's privacy and confidentiality. Documents pertinent information in the electronic medical record. Assists with coordination of all services for the patient in the Clinics. Works with the patient, significant other and health care team to establish patient centered short- and long-term goals. Demonstrates the knowledge and skill enough to assist with procedures and reusable medical equipment as permitted by approved local facility policies and procedures. Observation and documentation will include clinical reminders and the reporting of any noted change in the patient's condition to the RN. GS-4:(a) Six months of qualifying experience as an LPN or LVN; OR(b) Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; OR(c) Graduation from an approved school of at least 24 months duration. In addition to the required experience, the candidate must demonstrate all the following KSAs:(b) Provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and/or prescribed treatment regimes.
