services across the continuum of careKey responsibilitiesConducts emergency psychiatric assessments Interprets the psychiatric and psychosocial needs of the patients and gathers collateral information to determine appropriate disposition Conducts advocacy referral and discharge planning to meet patientfamily needs Works with patientsrsquo community behavioral health and medical providers to assure coordination of clinical careFacilitates treatment process by referring patientfamily members to appropriate level of care and other resources as well as providing direct crisis intervention servicesCoordinates care and dispositions of psychiatric patients with Emergency Department staff while working effectively as a team member both within and across clinical setting to provide an
