Qualifications:
Degree/Education: Master of Social Work (M.S.W.) degree from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).Life Support Certifications: Basic Life Support (BLS) certification.Experience: One (1) year within the past five (5) years in the independent practice of clinical social work/healthcare management for adults, children, families, seniors, or groups.Licensure/Registration: Possess a current, unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in any one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands.
Duties:
Coordinate appropriate referrals to community and other Government agencies for services.Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan, identifying the patient’s problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.Independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diverse backgrounds.Use medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.Implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals.Independently provide counseling and/or psychotherapy services to individuals, groups, couples, and families. Must practice within the bounds of the license or certification.Serves as Mental Health Case Management Function Leader; supervises staff in the performance of case management functions; assigns and manages staff caseloadsProvides clinical supervision to unlicensed providers of care and mental health technicians.Participates in the orientation and training of other staff.Serves on committees, work groups, and task forces at the facility.Completes medical record documentation and coding, and designated tracking logs and data reporting as required by local MTF/AF/DoD instructions, policies, and guidance.Provides care coordination and follow up planning for individuals being transitioned from inpatient psychiatric units and/or follow up from an emergency department visit or medical hospital admission following a non-fatal suicide attempt.
