POSITION SUMMARY:

The Framing Fatherhood Lead Case Manager will supervise three Case Managers and reports to  the Framing Fatherhood Program Director. The Lead Case Manager will be responsible for  administering direct service interventions for program participants. Services will be delivered in  accordance with program policies. The Lead Case Manager will deliver intensive case  management services via a one-on-one, trauma-informed, strengths-based service delivery  model. Responsibilities also include screening participants for risk factors, identification of  employment challenges and subsequent training and resource needs; general career counseling;  tracking of participant data; referrals to supportive services and counseling; and ensuring success  in overcoming challenges to achieve program goals. This highly visible leader will be adept at  relationship building, program development, and embodies the fatherhood qualities that are  representative of the passion, warmth, vision, intensity and love all fathers have for their  families.

Duties and Responsibilities:

Supervise Case Managers to ensure they provide high-quality, client-centered services,  including comprehensive assessments, individualized case plans, and ongoing support for  fathers enrolled in the program. 

Monitor and review case management activities, ensuring compliance with program and  funder guidelines, policies, and procedures. 

Provide regular coaching, guidance, and professional development opportunities to Case  Managers, fostering continuous improvement in service delivery. 

Provide direct services to assigned participants, including intake, development of a  service plan, referrals, advocacy, weekly case updates, tracking and recognition of  milestones, exit summaries, and follow-up. 

Collect and document program intervention data and program activities in the internal  tracking system. 

Facilitate and lead Case Conferences designed to review complex cases, discuss client  progress, and develop collaborative solutions to address any barriers to success.

Build on individuals' strengths to teach and model skills aimed at improving family  functioning in the areas of parenting, communication, and anger/crisis management. 

Understand and integrate tenets of healthy marriages, relationships, and economic  stability to offer comprehensive support to male participants. 

Support the recruitment of new participants into the program through individual and team  outreach. 

Support participant retention through the completion of all program components and  ensure continued engagement after graduation. 

Maintain familiarity with available program resources, both on- and off-site. 

Operate cooperatively as part of a multi-disciplinary team. 

Create new linkages with community-based organizations through the case management  process. 

Meet regularly with the Program Director for case reviews, staff development, and  program and professional development. 

Attend meetings, trainings, and conferences that contribute to personal development and  the growth and improvement of the Framing Fatherhood program. 

Perform other duties as required.


REQUIREMENTS: 

Master's degree in social work or related field required. 

Minimum three years of progressive employment experience with case management and supervisory experience. 

Experience in father involvement education, case management, maternal and child health,  infant mental health and/or mental health in a nonprofit setting preferred. 

Bilingual candidates preferred. 

Competence in providing health education and supportive services to adults. 

Strong oral and written communication skills. 

Experience in building collaborations. 

Ability to work with and relate well to other people and organizations. 

Must have and maintain a valid Driver's License in the state of his/her residence. 

Must have and maintain automobile insurance if driving a personal vehicle.
