Scope of Responsibilities: Works under supervision of the Manager of Kinney Home Services. Follows standard procedures to accomplish assigned tasks.  

  

 

Job Summary: Work on site in hospital serving as primary liaison of the Kinney Home Services. Assisting the Manager of Kinney Home Services to facilitate patients discharge needs with the Kinney Home Services entities. 

Responsibilities

Job Duties:   

  

Identify patients to the discharge and delivery programs by attending discharge meetings to confirm. Coordinating daily with the medical unit to develop an updated list of patients’ admissions.  Populate all required patients’ needs on the Kinney Home Services site. Meet with newly admitted patients to develop relationships, evaluate possible needs for discharge. Assist in the patient precertification process relating to commercial insurance or government health plans. Advocate with patients, patient families, and caregivers in the health care facility.  Address any queries or concerns from patient family and caregivers. Assists with ensuring adequate scheduling of all medications, durable medical equipment, and coordination of in-home services if necessary. Work closely with Kinney Home Services agencies to ensure timely delivery of discharge needs. Follows instructions and completes tasks with a sense of urgency. Assess the workload, establish the Kinney Home Services products pick up time and delivery. Assist in the delivery of Kinney Home Services products to the patient, including obtaining patient signatures. Provide quality customer service to all customers.  Maintain confidentiality regarding all patient information.  Must keep current with all State and Federal requirements related to conditions of employment.  Responsible for completing all mandatory and regulatory training programs. Travel required between facilities. Perform other duties as assigned.  Qualifications

Education:

Minimum:  High School Diploma (or currently enrolled) or GED Preferred:  AS Degree or Higher  

  

Experience Preferred: 3 years Hospital or Healthcare experience  

   

Special Conditions of Employment  

Drug Test Initial and continuous exclusion and sanction/disciplinary monitoring  

Compensation:

 

$17.00-$19.00 per hour

 

The final offer will be determined after careful consideration of multiple factors such as relevant skills, years of experience and education.
