Description
Basic Function/Overall Responsibilities: The Case Planner works collaboratively with the multi-disciplinary team to develop, implement and regularly evaluate/monitor the comprehensive case management plan, family assessment service plan, and discharge plan. The Case Planner will facilitate productive working relationships between families, agency personnel and other involved service providers. Flexible work schedule: non-traditional working hours required, on call as needed.
Principal Assigned Responsibilities:
+ Ensure that referring/custodial agencies and parents are kept closely informed of case movement, case planning decisions and significant events in the life of the youth on an ongoing basis.
+ Monitor the youth's daily functioning to ensure continuity of service in accordance with the individualized treatment plan and staff participation in the development and implementation of that.
+ Coordinate treatment efforts with other treatment team members via staff meetings, medication reviews, and other collateral contacts as deemed necessary.
+ Facilitate collateral contacts with individuals, schools and agencies outside the agency relevant to the youth’s clinical needs.
+ Accompany clients to court, when necessary, and provide advocacy or testimony, as appropriate.
+ Complete family assessment service plans (FASP) or Juvenile Justice youth and his/her families as appropriate to provide guidance and assistance with meeting goals and objectives that will change behavior and restore safety and well-being to Juvenile Justice youth and their family as appropriate.
+ Provide information, referrals, and assistance with linkage to appropriate clinical, financial, medical or other support services designed to fully integrate the family with community-based supports in accordance with the family service plan.
+ Maintain required documentation associated with the provision of services in a timely and accurate manner via the agency’s Electronic Health Record (EHR) and as required by relevant regulatory bodies, e.g. OCFS Connections system.
+ Responsible for providing individual and/or group skill building experiences for Youth.
+ Transportation for youth to community meetings, court appointments and home visits as appointed.
+ Attend agency in-service trainings, workshops and seminars as appropriate or required by supervisor.
+ Other associated tasks as deemed appropriate by supervisor.
Qualifications
Education and Experience Requirements:
+ Bachelor's degree, preferably in human services or a related field, required.
+ Minimum of one-year successful human service experience in residential or human services setting, preferably working with youth involved with juvenile justice.
