Provides targeted case management services to ensure mental health consumers have access to all services that are necessary for tenure and progress in the community. Performs duties in support of the Medical Center mission to ensure the highest quality of client care in an economically sound and efficient manner. This position serves: Adult consumers 18 years of age and up.
* Establishes and maintains working relationships with consumers and other staff as evidenced by a full caseload, chart review and through supervision. * Conducts outreach to consumers in their natural environment by being out in the field 75% of the time as indicated in relevant statistical reports. * Supports consumers in efforts to attain entitlements and housing evidenced by reports and supervisor’s observation. * Maintains clinical record of services and ensures compliance with standards as demonstrated by periodic chart reviews.
Provides effective problem solving, crises intervention and supportive counseling to clients, families, and community agencies and agents, evidenced by individual supervision, family feedback and service documentation. *Provides effective problem solving, crises intervention and supportive counseling to clients, families, and community agencies and agents, evidenced by individual supervision, family feedback and service documentation. * Assists medical needs of consumers by ensuring that yearly lab work is documented in the record.
*Assists clients to be seen by PCP annually * Participates in at least 5 days of continuing education to enhance skill development and maintain a record of Inservice and Continuing Education
* Participates in Quality Assessment and Improvement process by attending monthly meetings and demonstrated by attendance record. *Provides 24 on-call service as scheduled, evidenced by Crisis reports, On-Call schedule and feedback from clients. * Ensures accessibility to consumers and others by having cell phone in working condition and available at all times as evidenced by employee's record of responsiveness.
* Ensures continuity of care by informing consumers, family and concerned others of back-up system phone number based on documentation in chart * Delivers services according to a plan as evidenced by service plan in chart and updated every 6 months. * Works as a member of a team to benefit consumers and staff by attending staff meetings as evidenced by attendance record.
* Provides needed services to consumers outside of normal business hours. Keeps On-Call Intensive Case Manager aware of potential problems daily as evidenced by information in log. *Meets with all targeted case management consumers on a regular basis as evidenced by a documented contract every 14 or 60 days depending on level of care determination. *Understands confidentiality of records and consumer information. Complies with all areas of HIPPA.
Education and Training:
Bachelor Degree in Social Sciences Sociology, Social Welfare, Psychology, Gerontology or other social sciences. Bachelor’s Degree in other field with 12 credits in social sciences. A registered nurse; Working knowledge of Community Services, Entitlement Programs and Managed Care plans required. Associates Degree in Social Sciences and two years experience with direct contact working with mental health
Valid Motor Vehicle driver's license required.
