Intensive Care Coordinator - Bachelor's Level
Exciting opportunity to join our innovative and growing children's mental health program as a Bachelor Level Intensive Care Coordinator!

Responsibilities: - Provide comprehensive care coordination to children and families. - Perform initial clinical assessments to identify family and youth's need and goals. - Develop, implement and monitor individualized care plans based on family needs and goals. - Facilitate care plan team meetings. - Act as a liaison with collateral agencies and programs - Advocate for families and educate them on self-advocacy - Identify and assist youth and families in accessing necessary services - Ensure proper documentation, and safety planning - Participate in staff meetings, supervision, and training We offer an amazing orientation and well structured training in high fidelity wraparound!
