Salary: 18.96-28.44 USD Facility: LIFE PACE OLV Shift: Shift 1 Status: Full Time FTE: 1.000000 Bargaining Unit: ACE Associates Exempt from Overtime: Exempt: No Work Schedule: Days Hours: 8:30am-4:30pm Summary: The primary purpose of this position is to increase enrollment of potential participants to Catholic Health's LIFE Program, a Program for All-Inclusive Care for the Elderly program while incorporating the philosophy and mission of Catholic Health. This position will be responsible for receiving pre-enrollment information from individuals who express an interest in the LIFE program. This includes providing an explanation of LIFE, eligibility requirements, the philosophy and mission of the program as well as the services provided. Conducts facility tours, in-home intake assessments and program education. Works collaboratively with Community Liaison to build relationships with community referral sources and other key stakeholders. Must possess strong interpersonal communication and customer service skills related to a geriatric population. Responsibilities: EDUCATION + Associates Degree or a minimum of five (5) years of experience in relevant field required + Bachelor's Degree preferred EXPERIENCE + Two (2) years of healthcare/social services/human services preferred + Two (2) years of customer service or sales experience required + One (1) year of experience working with frail elderly population preferred KNOWLEDGE, SKILL AND ABILITY + Working relationship/knowledge of senior health issues and the Program's service area + Fluency in Spanish preferred + Must possess a valid driver's license and have a car available for work use + Strong computer skills required including word processing, spreadsheets, presentations, and data base management + Willingness to learn new software applications + Strong written, verbal and interpersonal communication skills + Ability to work independently. + Ability work productively with all levels of personnel including physicians, families and care givers + Customer focus + Accountable/Results oriented + Integrity and respect for others + Possess active listening skills + Demonstrated commitment to work within the team of health professionals REQNUMBER: 32202
