Intake Coordinator
Overview Job Description: The major function of this position is to provide high-level operational support for the UF-HBWP` clinical intake team to ensure goal achievements. This position will work with the Intake Team on screening complex patients / clients who may be eligible for the program and or are selected for the program within the UF-HBWP. Will work within a comprehensive interdisciplinary medical team to assess patient needs. Will support the initial and subsequent screening process to identify at risk social and behavioral patients entering the UF-HBWP. The position will support coordination of referrals to available resources within the community to further address medical, mental health, behavioral health, and social needs of the patient as needed. Responsibilities Job Requirement: Answer all incoming calls to the main Brain Health Wellness line and conduct initial cursory screening for determination of eligibility, phone screen and/or other actions which may be necessary in addition to registration of patients in Electronic Medical Record, in addition to Providing resources to callers not eligible for MIBH program. Outstanding customer service skills and proficiency in Microsoft Word, Excel, PowerPoint, Access Database and Outlook is necessary, in addition to a working knowledge of desktop computer operations. Experience using EPIC, REDCap and Formstack is preferred, in addition to some knowledge of the Military Culture. Inclusive of assisting patients with the completion of the program application and assessments. Obtain and track all necessary documents for prospective patients during the screening process. Obtain necessary medical records in concert with LCSW for all prospective patients in addition to assisting with uploading of critical records, as well as serving as back-up for coordinating patient care and scheduling appointments with internal UF-HBWP clinicians. Responsible for completing, sending, processing, and tracking all request for Releases of Information (ROI) forms, in addition to processing any urgent medical record requests during the 3-day evaluation phase of the program. Work with other teams to support the implementation of clinical office operating policies and processes. Supports patients/clients' discharge with the clinical team; supports the reviews and coordination discharge and post discharge plan requirements; orientations, training and provides resource support to family members as appropriate. Link participants to veteran service organizations and other community services based on participant's ongoing Care Plan. Some knowledge of neurology, TBI, psychology, rehabilitation, or some related clinical discipline. Ability to communicate effectively both in writing and orally. The ability to establish and maintain effective working relationships with employees at all levels throughout the institution inclusive of administrative and clinical settings. Qualifications Qualification: Experience Requirements: 2 years Practical experience required 2 years Working with Military or patients with TBI, PTSD, PH preferred 2 years Professional level experience working in clinical and administrative operations preferred Education: Bachelor's biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution preferred Additional Details: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.
