*$1,500 Sign On Bonus*Job Summary:

The Intake Coordinator is responsible for ensuring the highest level of service in the admission of new patients into hospice care.

Qualifications: High school diploma required; some college or bachelor’s degree preferred.A current LPN license in the state.
Minimum of 1-3 years experience in the health care field; hospice preferred. Excellent written and verbal communication skills. Excellent interpersonal skills. Microsoft Office products and database software experience required. Knowledge of managed care and Medicare/Medicaid reimbursement. Strong organizational and project management abilities. Interacts well with physicians, case managers, discharge planners, etc.8:00 am - 5:00 pm shift Monday - Friday. Responsibilities: Coordinates assessment and admission of all potential patients. Takes all initial phone inquiries regarding patient admission. Arranges for assessment of potential patients by a Homestead Hospice representative. Negotiates financial arrangements with insurance providers and documents the terms of the agreement. Communicates all billing information to the corporate billing department. Communicates insurance information and needs for an update to the Homestead Hospice billing department. Processes Medicaid/Medicare admissions. Provides clinical coordinator with information and dates for recertification. Arranges for the completion of all paperwork required for admission (Orders, certificate of terminal illness, legal documents, Medicaid forms, etc.). Notifies referral source of admission and or non-admission. Tracks all patients that are not taken under care (NTUC’s) and follows up to affect possible future admissions. Other duties and responsibilities as assigned.
