Reporting directly to the Lead APRN, this chaplain will play a critical role in addressing spiritual, emotional, and psychosocial needs for those dealing with chronic illness, end-of-life care, and distress. The chaplain will work collaboratively as part of the palliative care team, integrating spiritual care into patient treatment plans and participating in interdisciplinary meetings. *Responsibilities:* 1. *Spiritual Care and Support:* o Conduct spiritual assessments and interventions for patients experiencing distress due to illness or end-of-life concerns. o Provide compassionate spiritual care to patients and families in the hospital setting, ensuring support for those from diverse faith traditions and backgrounds. o Facilitate family meetings focused on goals of care and assist in care planning discussions with the palliative care team. o Maintain and document all spiritual care interventions in electronic medical records. 2. *Collaboration with Palliative Care Team:* o Work under the direction of the Lead APRN, collaborating with the interdisciplinary palliative care team to develop and implement comprehensive care plans. o Participate in daily rounds and team meetings, contributing to discussions related to patient care, symptom management, and family needs. o Collaborate with other chaplains and community clergy to ensure continuity of spiritual care for patients across different care settings. 3. *Patient and Family Counseling:* o Provide pastoral care and counseling to patients and families navigating chronic illness, grief, and end-of-life issues. o Assist in addressing anticipatory grief, spiritual distress, and family dynamics, supporting patients and families during critical moments. o Help patients and families explore their spiritual beliefs as part of their healthcare decision-making process. 4. *Education and Support for Staff:* o Serve as a resource to hospital staff, providing education about the spiritual and religious needs of patients. o Foster a team culture that emphasizes spiritual care as a core component of patient well-being. o Support interdisciplinary education sessions to help staff better understand the role of spiritual care in palliative settings. 5. *Professional Development:* o Continue to enhance knowledge and skills to meet the standards set forth by the Association of Professional Chaplains (APC). o Maintain professional affiliation with a recognized denomination and certification body. o Participate in ongoing professional growth through education, research, and adherence to best practices in palliative care chaplaincy. *Qualifications:* * *Education:* o Master of Divinity or Master’s degree in Theology, Religious Studies, or a related area from an accredited institution (CHEA accredited). o Four units of Clinical Pastoral Education (CPE) required. * *Experience:* o Minimum of two years of chaplaincy experience, with at least one year in a palliative care or hospital setting preferred. o Experience with chronic illness, end-of-life care, or hospice patients is essential. * *Licensure and Certification:* o Board certification (or eligibility) with the National Association of Professional Chaplains (APC), National Association of Catholic Chaplains (NACC), or equivalent certifying body. o If ordained, must be in good standing with the ordaining religious body. * *Skills and Abilities:* o Ability to provide spiritual care to people of diverse religious, spiritual, and cultural backgrounds. o Strong communication, interpersonal, and emotional support skills. o Familiarity with electronic medical records and basic computer skills. *Physical Demands:* * May be exposed to infectious diseases. * Ability to work under high to moderate stress environments and collaborate with diverse teams. *Preferred Qualifications:* * Experience working in palliative care settings. * Strong team-building qualities, with the ability to handle complex clinical cases and provide spiritual assessments for critically ill patients.
