Job Description

Candidates must have a minimum of 3 years of RN practice experience and some NICU practice experience to receive an interview.
 

Job Duties:

Provides expert care coordination, education, consultation and advocacy.Works with physicians, surgeons, consulting services, and hospital personnel to develop an organized care plan for patients who have been identified to participate in the program. Acts as the primary resource for care coordination for families.  Completes assessments, identifies goals and objectives for performance improvements in order to provide the best patient care, optimum customer service and improve overall satisfaction. Coordinates the specific program activities for the patient population. Works collaboratively with the clinical case manager, social work, outside agencies and families to coordinate care of the complex patient.Transitions the patient from the inpatient to the outpatient setting. Supporting patient education for theclinical diagnosis as well as educating families on community resources which are available.Coordinates a patient hand-off to the Lurie Children’s Coordinated Care Entity when appropriate.Documents all team, physician and patient/family communication and concerns pertaining to coordination of care and services in the medical record.Communicates essential information to patients, families, and members of the care team. Maintains a clinical database to develop and track key performance indicators. Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others.Other job functions as assigned.

Minimum Qualifications:

A BSN degree from an accredited School of Nursing is required.  A master’s degree from an accredited school of nursing is preferred.  Current licensure as a registered professional nurse in the State of Illinois is required.  Minimum of 3 years relevant clinical experience, preferably in case management. The ability to effectively communicate with patients/families, physicians, hospital staff, social agencies, school personnel and community resources.  Excellent organizational skills, including ability to effectively manage multiple competing responsibilities.Previous successful experience in new program development, coordination, and marketing. High level of initiative and autonomy.Proficient in a variety of computer programs, including Word, Excel, and PowerPoint and working knowledge of EPIC.

Pay Range

$83,200.00-$137,280.00 Salary
