As the Bilingual Lead Care Manager you’ll collaborate with youth, families, and care teams to craft individualized care plans, assess for resources and services, and facilitate communication among community partners as needed. You will play a pivotal role in educating youth and families about available supports and accompanying them to appointments when needed to ensure they feel empowered and informed.
ABOUT ENHANCED CARE MANAGEMENT
The Enhanced Care Management (ECM) program is a Medi-Cal managed care benefit that addresses clinical and non-clinical needs of high-need youth and families through the coordination of services, connection to community resources, and comprehensive care management. Team members work with youth and families providing services almost exclusively in the community: in families’ homes, schools, other community locations that's within a 90-mile radius of the program office.

Responsibilities Conduct outreach to enroll new clients in ECM, establishing rapport and trustCoordinate care activities and maintain communication with external providers including county substance use disorder and specialty mental health providers as appropriateCollaborate with youth, families, and care teams to assess needs and develop comprehensive care plansManage referrals, coordination, and follow-up to needed services and supportsProvide education and identify support needs for youth and their familiesBuild and foster collaborative relationships with community resourcing agenciesProvide services in-person and accompany clients to appointments when neededComplete required documentation in a timely manner QualificationsREQUIRED:
Experience/education requirements:Bachelor’s degree in Social Work/Psychology/Counseling + 1 years of experience ORAssociate’s degree or equivalent + 2 year of applicable work experience ORHigh school diploma/GED + 3 years of applicable work experienceExperience required working with families who are experiencing the impact of complex systematic disparities that contribute to community and individual mental health challengesBilinugal Spanish skill set requiredFlexible schedule with the ability to work some evening hours or earlier mornings, scheduled ahead of time as neededMust be at least 21 years of ageValid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policyTB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirementsProof of up-to-date COVID-19 vaccinationPREFERRED:
