Humana Healthy Horizons in Virginia is seeking Housing Specialists (Care Management Support Professional 1) to support the clinical care teams to help Medicaid members find and keep, safe and stable housing. They will work with Commonwealth wide and local housing entities to find appropriate housing services and supports for Virginia Cardinal Care members, including ensuring that transitioning members from facility to community are connected to appropriate housing resources. This virtual team mainly performs moderately complex administrative support tasks for the state of Virginia Medicaid program.
+ Collaborates with the Transition Care Coordinators and ensuring that members transitioning from facility to community are connected to appropriate housing resources, including but not limited to, referrals to local, Commonwealth and federal supports for permanent housing.
+ Works with housing agencies to help develop and access affordable housing services for members receiving Long-Term Services and Supports (LTSS).
+ Educates and assists Care/Service Coordinators and network providers regarding affordable housing services.
+ Consults directly with members or authorized representative to identify safe, affordable housing, based upon member needs.
+ Identifies and maintains a list of emergency, transitional, and permanent housing resources throughout the state.
+ Helps build innovative partnerships between Humana and housing-related community-based organizations to improve member health outcomes.
+ Attends community-based housing organizational meetings as needed.
+ Advocates for members with landlords and housing agencies as needed.
+ Coordinates with community organizations, government agencies, and property owners to secure living arrangements, negotiate rent, deposits, and move-in terms on behalf of members.
+ Serves as a liaison between Humana and housing agencies, community-based organizations, and providers regarding issues related to housing needs for members.
+ Coordinates with Humana’s housing strategy partners.
+ Collects and analyzes performance reports on Humana’s housing support functions to monitor adherence with benchmarks, identify opportunities for process improvement, and develop recommendations for leadership.

**Required Qualifications**
+ **Must reside in the Commonwealth of Virginia.**
+ Two (2) years of experience working for a public agency or related work in social services, community services, housing support, or case management fields.
+ Prior experience in supporting older adults and/or persons with disabilities to secure accessible, affordable housing through federal and local programs including HUD subsidized housing and voucher programs, public housing authorities, the Homeless Management Information System (HMIS), and USDA’s Rural Development Single Family and Multi-Family programs.
+ Intermediate working knowledge using Microsoft Office Programs specifically Excel, PowerPoint, Outlook and Word.
+ Knowledge of, and expertise in, commonwealth-specific demographics, organizations, housing, and geography.
+ Demonstrated ability to constructively work with persons in crisis situations.
+ Exceptional communication & interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.
+ This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
+ Ability to travel throughout Medicaid regions in the Commonwealth of Virginia.
+ This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
**Preferred Qualifications**
+ Associate’s degree.
+ Knowledge of and experience with relevant Housing and Urban Development (HUD) regulations and affordable housing financing.
+ Previous experience tracking, analyzing, and reporting on Commonwealth and Federal changes to housing and community development programs.
+ Knowledge of and experience with computerized documentation systems.
+ Bilingual or Multilingual: English/Spanish, Arabic, Vietnamese, Amharic, Urdu or other - Must be able to speak, read and write in both languages without limitations and assistance. _See “Additional Information” section for more information._
**Additional Information**
+ **Workstyle:** **Workstyle:** Field - This is a field position where employees perform their core duties at non-company locations, such as providing services at business partner facilities or prospects' and members' homes.
+ **Travel:** 50 - 75% field interactions with members, and their families and providers. May need to attend onsite meetings occasionally in Humana Healthy Horizons office in Glen Allen, VA.
+ **Workdays and Hours:** Monday – Friday; 8:00am – 5:00pm Eastern Standard Time (EST).
+ **Language Assessment Statement:** Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

**Pay Range**
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$45,400 - $61,300 per year
