Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness. But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home. Breaking Ground’s Street to Home program s makes regular contact with more than 2,000 street homeless individuals and connects more than 500 individuals to housing, medical and mental health services, substance abuse counseling, and other essential supports each year. The caring, individually tailored attention clients receive at each stage of their journey from street to home ensures that more than 95% of people who Breaking Ground places remain stably housed. The Housing and Reporting Specialist will help achieve program targets by expediting the housing placement process by assisting clients in achieving housing readiness, establishing eligibility profile s , seeking out and matching permanent housing opportunities. In coordination with the Housing Specialist and case management team help complete and submit housing applications and secure housing interviews. Must be able to assist with mock interviews, conduct housing related groups, escort client s to appointments, and assist with move-ins. Additionally, the Housing and Reporting Specialist will assist the Program Leadership in ensuring program wide reporting compliance with both funder reporting and interagency reporting. This individual will be the program's point per son for the following databases: Breaking Ground's internal database CDB , Department of Homeless Services (DHS) StreetSmart , and Manhattan Outreach Consortium (MOC) MOCapp . ESSENTIAL DUTIES AND RESPONSIBILITIES: + Ensure clients are prepared for the housing process by conducting mock interviews, groups, escorts to housing interviews, and ensuring that all necessary documents for housing applications have been secured + Help prepare and review HRA 2010e and other appropriate housing applications + Clearly communicate with case management staff regarding upcoming interviews, sent applications, and follow-up with housing providers, via progress notes, staff meetings and weekly supervision + Track clients progress towards housing readiness + Identify and maintain a wide array of housing options and resources, including building relationships with fair market landlords and rented rooms when appropriate + Ensure that clients are matched and placed in housing appropriate to their needs + Escort clients to appointments as needed + Complete and submit required reports and data for both internal requests and formal reports to the program's funders + Maintain, train, and work with staff on ensuring StreetSmart and CDB are being probably utilized + Work with Program Leadership to ensure contract compliance, data analysis, and reporting for the Street to Home program in Manhattan + Provide a level of internal QA to ensure that Street to Home Manhattan is meeting the minimum expectations regarding data entry for various performance metrics (ex: MOC Monthly Monitoring Reports) + Perform other related duties as assigned MINIMUM QUALIFICATIONS: + Bachelor’s degree or equivalent experience + Excellent interpersonal and problem-solving skills + Strong writing and verbal communication skills + Demonstrated proficiency with Microsoft Office (Word, Outlook, Excel) + Two years’ experience, preferably in a non-profit or real estate environment, knowledge of supportive housing and DHS housing programs preferred + Previous experience working with homeless/formerly homeless populations, adults with substance abuse /dual diagnosis preferred + Background in Data Analytics strongly preferred + B ilingual in Spanish and English preferred + E xperience with HRA 2010e, AWARDS /Foothold and CARES preferred + Familiarity with homeless/housing services in NYC preferred + Must have the ability to handle multiple tasks in a fast paced, dynamic team environment and willingness to shift easily between various responsibilities with diverse populations + Must be able to use public transportation to acc ompany clients to appointments + Must be willing and able to engage in moderate physical activity to engage wi th street homeless individuals ( Such activities include and are not limited to moderate walking and climbing ) EOE/M/F/Vet/Disabled Other details + Pay Type Hourly + Min Hiring Rate $25.85 + Max Hiring Rate $25.85 Apply Now + S2H Manhattan, 15 East 27th Street, New York, New York, United States of America
