We're seeking a dedicated Hospital Liaison Recovery Coordinator to provide a hospital- based service in Gateshead. You will work in close partnership with Accident & Emergency teams to develop and implement care plans for frequent hospital attenders.

 

Where: Gateshead

Full Time Hours: 37.5 per week

Full Time Salary: £27,449.52 - £31,529.41 (based on full time working hours, pro rata for part time)

Responsibilities

About the role:

Work in close partnership with A&E, RAID and Change Grow Live community teams to develop and implement care plans for frequent hospital attenders.To provide specialist assessment of patients attending/admitted to hospital with alcohol related issues using AUDIT C and AUDIT 10 screening tools. To provide brief interventions to patients scoring below 20 on AUDIT 10Work with community teams to ensure assertive and effective follow up of both repeat presentations and discharges from the hospitalEffectively communicate with GPs keeping them informed of patients status and discharge plan/referrals within 5 days of patient’s hospital dischargeEducate and support carers and contribute to the assessment of carers’ needsDeliver provision of triage and/or comprehensive assessments in hospital and to support to ensure patients ensure with Change Grow Live

About you:

Training and/or experience in psychosocial intervention (e.g. psychology or counselling graduate)Be an excellent communicator, both verbal and written with good IT skillsProactive and able to work on own initiative with excellent time management and prioritising skillsHave a good working knowledge of mental health interventions, services, and good practiceBe a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholdersAbility to manage change successfully in a way that prioritises the needs of service usersUnderstand the importance of information governance processes and commit to follow and apply all necessary safeguardsSeek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of othersAbility to manage challenging behaviour in a calm and professional manner.
