Hospice - LVN - LPN San Antonio, TX (http://maps.google.com/maps?q=4538+Centerview+Dr.+San+Antonio+TX+USA+78228) Job Type Full-time, Part-time Description Klarus Hospice / Jet Health, Inc.is looking for afull time / part time or PRN / Per DiemLicensed Vocational Nurses (LPN / LVN)to join our dedicated team to provide community-based care for our patients currently receiving hospice services in theSan Antonio, TXmetro area. The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes. Requirements + Two years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Hospice experience highly preferred. + Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills. + Home Care Home Base knowledge highly preferred + Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse. + Coordinate care with the interdisciplinary group, patient/family, and referring agency. + Assist in the development and implementation of patient care plans. + Monitor patient health and report any changes to the supervising RN. + Administer medications and treatments as prescribed. + Educate patients and their families on health care needs, conditions, and options. + Maintain accurate and timely documentation of patient care and progress. + Graduate of an accredited School of Vocational/Practical Nursing. + Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license. + Reliable transportation and valid, current auto liability insurance. + Work in patients' homes in various conditions. + Ability to work a flexible schedule and travel locally. + Ability to lift up to 50 lbs. and move patients. + Ability to make quick decisions and acquire necessary resources. + Must meet patient/family individualized psychosocial needs.
