Position Summary

The Home Health Intake Supervisor is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Intake Supervisor's primary responsibility is to coordinate and manage all referral management activities within their team. S/he assumes responsibility for continuity, appropriateness, and quality of service delivered by Compassus.


Position Specific Responsibilities

Leads, mentors, and motivates team of Home Health Admission Coordinators.Supervises and provides guidance, support, and ongoing training to clinical & non-clinical team members to ensure they meet performance standards and goals.Performs regular patient chart audits, generates reports, and analyzes referral data to identify trends, areas for improvement, and opportunities for efficiency.Makes recommendations for process enhancements based on data analysis.Holds monthly team meetings and KPI performance reviews.Oversees the referral intake process, ensuring timely and accurate processing of patient referrals.Reviews and assesses referral documentation for completeness and compliance with relevant regulations.Collaborates with agency leadership, case managers, and growth leaders to obtain necessary information for referrals.Stays up-to-date with regulatory guidelines and ensures all referrals adhere to legal and compliance standards.Maintains detailed records and documentation related to referral management and team performance.Coaches employees, provides input for performance evaluations, counsels and provides disciplinary actions to assigned personnel in conjunction with the manager, and works to facilitate individual and team development that drives positive results.Performs other duties as assigned.


Education and/or Experience

Associate or Bachelor's degree in Nursing required.Minimum of two (2) years of experience as a registered nurse and one (1) year in home care required.Prior supervisory experience preferred.


Skills

Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.


Certifications, Licenses, and Registrations

Active and unencumbered Multistate Compact Registered Nurse license, Michigan Registered Nurse license, and license in any other state(s) where Ascension at Home Together with Compassus operates required.


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
