Responsibilities
Position Summary
• Responsible for preparing and scanning documents into the electronic document
management system, viewing scanned documents for image quality and accuracy;
reviewing, identifying and monitoring documentation requirements for electronic
medical records.
Job Roles
• Safety
o Manages processes effectively in regard to employee/patient safety.
• Record Keeping
o Maintains all required records, reports, statistics, logs, files and other
documents as required.
• Process Improvement
o Promotes a culture of process improvement by participating in
unit/department based programs that supports the system’s process
improvement goals.
o Actively participates on system-wide or hospital-based teams as needed.
• Role Specific Responsibilities
o Assemble records in the hospital defined order.
o Determines the readiness of the documents for quality control process.
o Maintains scanning equipment to assure quality of scanned documents.
o Performs functions according to quality and timeliness guidelines.
o Retrieves / Receives patient’s documents.
o Reviews and corrects batch images
o Uploads corrected batch.
o Validates index values.
o Collaborates with Patient Identity Specialist regarding physician
delinquency status.
o Monitors and corrects chart deficiencies, rejected deficiencies and chart
completion regarding medical Staff Rules and Regulations.
o Serves as a liaison with physician and clinicians regarding use of
electronic medical record system.
o Maintains acceptable chart completion turn-around-time within the
revenue cycle.
o Updates clinician events.
QualificationsEducation
• High School Diploma
• Associates Degree preferred

• Technical Experience: 1 year
Physical Activities
• Intermittent hand manipulation required
• Intermittent lifting and carrying of 20 lbs.
