Healthier Community Grant Coordinator

The Healthy Communities Grant Coordinator supports grant-funded population health initiatives. The Grant Coordinator will liaise with members of the Healthy Communities team, Care Management, Nursing staff, Service Line leadership and community-based partners. The Grant Coordinator supports program planning, implementation and evaluation. They will schedule and facilitate team meetings, manage project work plans, coordinate staff training and education events; collect, analyze and report training evaluation data; support completion of grant reporting, and other tasks, as assigned. **Responsibilities** **Healthier Community Grant Coordinator** Provides project management to CHI Health hospitals and clinics in carrying out grant deliverables related to Healthy Communities grant programs. Ensures timely follow-up on assigned projects. Supports grant and budget management, processing grant expenditures, maintaining/ updating the grant budget and preparing midyear and annual reports to submit to the project funder. Convenes an interdisciplinary steering committee to plan, implement and evaluate grant- funded population health programs at CHI Health. Schedules meetings, facilitates discussion and captures notes. Liaises with health system and community partners to ensure grant deliverables are completed and comprehensive data for reporting / quality improvement is tracked and analyzed. Maintains privacy standards of protected health information (PHI). Works with clinical teams to define/ communicate inclusion criteria for program eligibility. In consultation with the steering team, refines and tracks key performance indicators to monitor program performance. Develops/ maintains subject matter expertise in Healthy Communities impact areas, such as maternal and infant health and homelessness. Coordinates training and education opportunities. Partners with Legal to draft and execute contracts with community-based organizations. **Qualifications** Bachelor's Degree in Business Management/Administration, Public Health, Health Administration and Policy or related field. Two years experience in public health, administration or health care delivery, or a combination of academic coursework in those areas and professional experience. **Pay Range** $20.62 - $28.35 /hour We are an equal opportunity/affirmative action employer.
