Health Information Specialist
Overview This role is part of the Health Information Services Department, a shared service of the Dartmouth Health health system that centers on the patient’s health (medical) record. The Health Information Specialist is responsible for ensuring health information is properly stored in a patient’s health record. The Health Info Specialist prepares and digitizes documents for scanning and indexing into the electronic health record. The Specialist reviews and evaluates the quality of scanned documents to preserve the integrity of the health record and ensures documentation is accurate, complete and meets legal, regulatory or accreditation requirements. Health Information Specialists also may perform introductory records management functions. Responsibilities * Prepares and processes documentation for scanning into the electronic medical record using a scanner and associated software application. * Indexes documents into the electronic medical record, per established procedures, accurately based on predefined EMR chart categories and standards. * Assures each document has patient identifiers before scanning and indexing to an electronic health record. * Manages digitized documents and incoming faxes via work queues to index patient documentation to correct health record or distribute to appropriate departments as needed. * Ensures digitized documentation is legible and complete before indexing. * Operates and monitors equipment to ensure proper functioning. Performs routine maintenance on scanning equipment per established procedure. * Performs straightforward records management functions, such as retrieving records from offsite vendors and pulling and reshelving from internal collections, as needed. * Maintains and safeguards all health information received in both electronic and paper form. * Follows company retention policy and departmental procedure for storing and purging paper copies of documentation that have been digitized. * Provides excellent customer service and displays professionalism both when interacting with internal/external customers and when responding to phone calls or email. * Works independently with self-discipline and works well in a team environment. * Maintains total confidentiality regarding all patient information. * Performs other duties as required or assigned. Qualifications * High school diploma or equivalent in education, training and experience. * Demonstrated experience handling confidential material. * Excellent attention to detail, organizational, interpersonal and communication skills required. * Understanding of medical terminology preferred. * Experience using a scanner and excellent keyboard skills preferred. * Knowledge of Spanish helpful.
