• Days 7a-3:30, or 8a-4:30. 40 hours
• Every other holiday and weekend rotations may apply.

RESPONSIBILITIES:
Reports to the Nurse Manager, in collaboration with the unit charge nurse and nursing staff. Expected to assist in the maximization of functional independence and morale of patients on the ACE unit and with dementia focusing on the patients' abilities.
Provision of positive emotional environments for dementia patients by recognizing and addressing their needs and promoting independence with ADLs' toileting, ambulation, and nutrition.
Forms a variety of technical tasks and responsibilities including direct patient care, assisting with hygiene,
ADLs, toileting, ambulation, and nutrition. Promotes and supports families caring for dementia patients to allow them the opportunity to have therapeutic relationships. Management has the right to change or add to job responsibilities at any time.
1) Develop and maintain good human relations skills. Consistently display a respectful, empathetic and caring attitude toward patients and families. Interact with members for the heath care team, RN, PCT, MD. etc. in a professional manner. Accept changes in assignments to accommodate the patient’s needs. Accept constructive criticism as demonstrated by positive behavioral changes if indicated.
2) Maintain a safe environment for the patient. Demonstrates knowledge and skill in maintaining a safe environment. Accompanies the patient off unit as directed by the nurse and remains in sight of the patient at all times in these areas, (i.e., healing garden, activity room). Transports patients in wheelchairs to these areas. reports safety hazards to the RN.
3) Provide age-appropriate care to patients. Demonstrate knowledge of developmental stages for the adult and geriatric age groups and applies developmental theories and concepts when implementing care for these patient populations.
4) Assist the patient with ADL's and hygiene needs when necessary and supporting and encouraging patients to be independent in their ADL's by allowing them to retain normal daily routines and respecting patient preferences. This includes setting up for ADL's , assistance with bathing, dressing and completing morning and evening routines. Ensure environment in the evening is suitable for rest.
5) Assists patients with ambulation assistance and assistive devices, within unit parameters. Documents as appropriate and communicates to the care team distance ambulated and frequency.
6) Assist and support with nutrition and feeding. This included setting up for meals, feeding and coaching.
7) Assistance with cognitive activities individually or in small groups and monitor for safety and unusual behaviors.
8) Participates in purposeful hourly rounding, ensuring environment is free of safety hazards, and assisting with repositioning, toileting needs and ensuring all necessary items, (phone, call bell, nourishment) is in reach. Assures TABS and bed alarms are on and activated when appropriate.
9) Responsible for the management of assistive device supplies, palliative care room supplies and maintenance of activity room and quiet room.
10) Participates in competency validations, educational activities and staff meetings Completes all mandatory in-services and web strategies 100% of time. Attends mandatory validations. Attends all additional programs as required. Continually seeks to improve skill level and knowledge base.
11) Communicates to patients they are a high priority even during busy times. Treats patient information as confidential. Displays sensitivity to patients through non-verbal communication. Utilizes body language such as eye contact, smiling and demonstrates interest in connecting with patients.
QUALIFICATIONS:
Previous customer service, experience as a direct health care provider, CNA, HHA, or PCA
Preferred Work Experience:
CNA experience preferred
