Gastroenterology Nurse Navigator

The Gastroenterology Nurse Navigator works under the supervision of the Clinical Manager. Assesses, plans, implements, evaluates, and supervises individual care to patients in both the acute and ambulatory settings. Serves as link between patients, providers, and other services needed for transition of care. Ensures appropriate education and support focused on teaching self-care behaviors. Accepts the responsibility for directing nursing care during transition period and ensures appropriate hand-off to care management as needed. Collaborates with acute and ambulatory nursing staff, care managers and providers.

 

The Gastroenterology Nurse Navigator oversees and coordinates the clinical department provider schedules as well as the schedules for the Ambulatory Surgery Center and the HMC-G Surgery Center scheduling for acute, in-patient procedures. Oversees the provider(s) GI practice.  Directs, educates and is a resource to authorization coordinators, nursing staff, and office personnel. Provides patient education to the patient for a variety of GI procedures and diagnoses. Performs Capsule Endoscopy (Pill Cam) procedures and documents these appropriately.

 

Education, Work Experience and Licensure:  

•    RN with a minimum of 5 years work experience in the field of Gastroenterology. 

•    Holds a current nursing license in good standing in the state of Ohio

•    Strong computer skills and ability to learn new clinical information/data warehouse/cloud-based reporting systems

•    Excellent verbal and written communication skills

•    Prior healthcare leadership skills desirable

•    Evidence of working independently and effectively with other healthcare professionals, patients, patient’s families, or support persons to achieve optimal patient results 

•    Obtain and maintain RN conscious sedation.

Education, Work Experience and Licensure:

 

· LPN or RN with a minimum of 5 years work experience in the field of Gastroenterology.

 

· Holds a current nursing license in good standing in the state of Ohio

 

· Strong computer skills and ability to learn new clinical information/data warehouse/cloud based reporting systems

 

· Excellent verbal and written communication skills

 

· Prior healthcare leadership skills desirable

 

· Evidence of working independently and effectively with other healthcare professionals, patients, patient’s families or support persons to achieve optimal patient results
